Our mission is to help individuals thrive at home by creating customized aging-in-place plans that honor independence, autonomy, and happiness at every stage of life.

Navigating the transition from hospital or rehabilitation back to home can feel overwhelming—especially when decisions need to be made quickly and with confidence. Our Discharge & Care Coordination service provides thoughtful, hands-on guidance to ensure nothing is missed and everything is aligned with your goals.
We work closely with you, your family, and your care team to create a clear, personalized plan for a safe and supported return home. From reviewing discharge instructions and medications to coordinating services, equipment, and follow-up care, every detail is managed with precision and care.
This service goes beyond logistics. It offers reassurance—knowing that each step has been carefully considered, communication is seamless, and support is already in place before you walk through the door.
The result is a smoother transition, reduced risk of complications or readmission, and the peace of mind that comes from having an experienced advocate guiding the process.

Our Home Safety Assessments are designed to provide the right level of guidance based on your needs—ranging from a focused safety review to comprehensive evaluation and planning.
Services may include an in-home walkthrough to identify immediate risks, practical recommendations to improve safety, and a broader assessment of health, medications, support systems, and daily living needs. For those requiring a more in-depth approach, assessments can also include a detailed written report, personalized recommendations, referrals, and guidance on available resources or payment options.
For homes that require physical adaptations, we also offer specialized planning for accessibility and modifications, with clear direction to support contractors or designers in creating a safer, more functional environment.
Whether you’re seeking reassurance, navigating a transition, or planning for long-term safety, each assessment is delivered with careful attention to detail and a clear path forward.

We provide expert consultations to senior housing communities and service providers navigating complex aging-in-place challenges. Our work addresses real-world issues such as supporting residents with geriatric syndromes, managing high-needs situations with limited resources, and applying best practices for those who need additional assistance but do not qualify for—or cannot afford—higher levels of care.
With over 20 years of experience across diverse economic and clinical populations, we deliver practical, field-tested guidance that balances resident dignity and autonomy with staff capacity and operational realities.
We partner with leadership and staff to problem-solve, strengthen supports, and implement realistic strategies that improve stability, safety, and quality of life—particularly in low-income senior housing settings.

At Mata Aging-in-Place Management Services, we believe everyone deserves the opportunity to age safely, independently, and with dignity—regardless of income or circumstances. Aging in place works best when the whole person is considered, and solutions are realistic, compassionate, and tailored to real life. Our work supports autonomy, informed choice, and practical strategies that help people remain at home and in their communities for as long as possible.
